Crescent Road is a three bedded residential care home located in the North Manchester Area. It provides 24 hour support, for adults with mental health and learning disabilities.

Services Offered

 

Before becoming a resident, a comprehensive assessment of needs, wants and wishes is undertaken. Part of this process will be a visit to the home and if the person is unable to visit themselves, we will ensure that a family member or advocate visits before any agreements are made about the placement. The service will be initially for a trial period. During this time a person will be allocated a key worker and together a person centred support plan will be created. This plan will be reviewed on a monthly basis. Crescent Road provides 24 hour support including one staff sleep. Individuals are supported by staff to undertake preparation of meals, household duties to assist them with all aspects of daily living.

Crescent Road is a large semi detached property, with garden areas to the front rear and side providing lots of out door space for individuals to use and enjoy. The home can accommodate up to 3 service users with all 3 bedrooms providing single occupancy. Ground floor bedroom provides an en-suite and wheel chair access. The property has a kitchen with a breakfast bar, 2 lounge areas, one bathroom communal, and two toilets communal.
