Park Road is ideal for people with Learning Disabilities who are ready to take the next step towards independent living, be that from residential care or a family setting. Two of the tenants at Park Road have recently moved onto their own flats in the community and Regard continues to provide outreach support. They have all made fantastic progress and achieved some very positive personal outcomes, in relation to independence and quality of life.
The Service
The team at Park Road provides high quality support which is tailored to reflect individual need. This is achieved through our ‘Outcome Focused Person-Centred Plans’ and we closely follow the Government’s ‘Valuing People’ criteria which ensure that we respect tenant’s rights, inclusion, choice, and independence at all times.
What we offer
- 24 hours a day shared support; which currently includes sleep-in support
- 1:1 support for specific identified assessed needs
- We can evidence reduction in 1:1 support hours as tenants gain new skills and greater independence
- Support is structured to ensure that the individual has the opportunity to access a full range of social activities within the community e.g. football, cinema, swimming and local clubs and where possible employment opportunities and access to local colleges
- A well motivated and enthusiastic trained staff team, trained in NVQ, supported living, epilepsy and all the core mandatory training requirements. Specialist training will be provided to meet individual assessed needs if required
- Support individuals in accessing appointments with all healthcare professionals
- We have positive working relationships with the local housing department, health professionals from community learning disability teams and families
- We provide outreach support for tenants now living locally in the community (whom once lived at Park Road)
Support
The following provides an example of what is deemed individual support and what is covered by the shared support:
Typical Individual Needs
- Prompting with personal care. (We can provide support from our registered Dom Care agency if required)
- Budget planning and finances
- Menu planning, shopping and cooking
- Social Activities/inclusion and community links
- Vocational and educational opportunities
- Administration of medication. Support with health facilitation
- Transport training where applicable to further promote independence
- Initial orientation to new surrounding and establishing new routines
- Planning and preparation of life skills for move onto independent living
Shared Support
- Supporting individuals to maintain their tenancies and live with others
- Emotional support/socialisation
- Promotion of group social
- Activities as desired by individuals
- Supervision with household, domestic duties and laundry
- Supervision with meal preparation
- Regular tenants meetings
- Supporting individuals to maintain the safety of their environment
- Benefits support and assistance with claims by our specialist Benefit Team at Head Office
- Promoting self advocacy, user involvement and support to access advocacy services
About the Property
Park Road is a Victorian house near to the centre of Sittingbourne and has all the community facilities one would expect to find in a town centre location including some supermarkets, shops, restaurants, pubs, a cinema and other leisure facilities. The home has excellent transport links direct to London, Maidstone, Ashford and coastal resorts.
Park Road has been split into two self-contained areas. Within the ground floor there are 3 fully furnished bedrooms, kitchen, bathroom, and living area. Upstairs, there are a further 6 furnished self-contained rooms with fully fitted kitchenettes. There is a communal lounge, bathroom, toilet, shower and laundry room.
