Gordon Avenue is a brand new Supported Living Service within the heart of Camberley and is unique in the model of supported living it can provide.  It is a younger person’s transition service ideal for young adults leaving residential college or foster care/ family home.  We will specalise in supporting younger adults with learning disabilities, autism, emotional needs and or underlying mental health needs. The service can accommodate five tenants.

 

Our aim is to support individuals to gain the skills necessary for a move onto independent living, if they so choose. We have the scope to provide outreach support once tenants have moved, thus maintaining consistency and continuity.

 

We promote enablement and inclusion at all times. By doing so, tenants have opportunity to explore new experiences and make lifestyle changes based on their own decisions, which previously, others have made for them. 

Staff support is flexible and structured around individuals’ needs on a daily basis and the staff team are recruited specifically to meet the skills and qualities of the people they are to support.

 

Staff will actively support and encourage individuals to attend college, seek employment both paid and voluntary and participate in numerous recreation opportunities available within local community. We have supported a number of tenants at Middle Gordon Rd to seek voluntary and or part time employment in specific areas of personal interest.

 

We have recently introduced an independent living skills assessment tool with tenants within our other local supported living service at Middle Gordon Road which is in easy read format and outcome driven.  The tools starts with an person centered assessment of the individuals skills and focuses on what skills are need to prepare for independence. Not only does it focus on skills for independence, it also covers topics such as health, relationships, employment socialization and many more. Feedback from tenants is positive and success can be measured by notable increase in tenants self esteem, confidence, emotional wellbeing and participation with daily living and social opportunities.  Our aim is replicate this success and user lead focus within Gordon Avenue.

 

 

About the property

 

Gordon Avenue is a semi-detached house, within walking distance of Camberley.

 

Located on the ground floor we have two bedrooms, a kitchen and a large lounge/dining area.

 

Three further bedrooms located on the first floor and a large bathroom.

 

Plans are in place to provide en-suite facilities within all bedrooms.

 

Plans are also in place to convert the garage into and independent training unit.

 

There is a decent sized garden at the rear of the property with BBQ area. This will become a hub of social activity during the summer months.

 

The home is within close proximity to shops, pharmacy, post office, pubs, church, Health Centre and parks. Public transport links to neighbouring towns and London are very good.

 

All tenants we will have opportunity to engage is social activities which will rage from: college, employment, drama, football, bowling, cinema, swimming, gym, shopping and the numerous social and recreational opportunities available within Surrey.

 

The Service

 

The team at Gordon Avenue will provide high quality support which is tailored to reflect individual need. This is achieved through our ‘Outcome Focused Person Centered Plans’ and we closely follow the Government’s ‘Valuing People’ criteria which ensures that we respect tenant’s rights, inclusion, choice, and independence at all times.

 

We Offer  

  • 24 hours a day shared support; which currently includes sleep-in support.
  • 1:1 support for specific identified assessed needs.
  • We will evidence reduction in 1:1 support hours as tenants gain new skills and greater independence.
  • Support is structured to ensure that the individual has the opportunity to access a full range of social activities within the community.
  • A well motivated and enthusiastic trained staff team, trained in NVQ, Supported Living, Epilepsy, Mental Health, Challenging Behavior, Autism and all the core mandatory training requirements. Specialist training will be provided to meet individual assessed need if required.
  • Support individuals in accessing appointments with all health care professionals.
  • We have positive working relationships with local housing department, Care Managers and Health Professionals from Community Learning disability teams.
  • We have the scope to provide outreach support for tenants living in community as we have an established domiciliary care agency supporting children and adults which is based in Camberley

  • Prompting with personal care. (we can provide support from our registered Dom Care agency if required).
  • Emotional support and confidence building required to support independence

  • Menu planning, shopping and cooking
  • Social Activities/inclusion and community links
  • Vocational and educational opportunities.
  • Administration of medication. Support with health facilitation. Promotion and awareness of health needs
  • Travel promotion where applicable to further promote independence
  • Initial orientation to new surrounding and establishing new routines
  • Planning and preparation of life skills for move onto independent living.
  • Support individuals to maintain their tenancies and live with others
  • Emotional support/socialisation
  • Promotion of social opportunities as desired by individuals
  • Supervision with household domestic duties and laundry
  • Supervision with meal preparation
  • Regular tenants meetings
  • Support individuals to maintain the safety of their environment
  • Benefits support and assistance with claims by our specialist benefit team at Head Office.
  • Promoting self advocacy, user involvement and support to access advocacy services.
