Rhyme House provides a new service to adults aged from 18-25 years with Learning Disabilities whom may present autistic tendencies and have specific needs. Not all the people we support here will want to move on from Rhyme House when they reach 25 years of age and we would be happy to continue a placement beyond this age.
The transition from care services and/or residential college is a difficult and emotional time for young people and their families. We aim to provide a seamless transition via a robust and transparent transition support plan, implemented in partnership with families, transition workers and other appropriate professionals.
Our commitment is to provide the highest possible quality of support to our Service Users. Our enabling, self-empowering approach is tailored to individual aspirations, ensuring a high
level of consumer satisfaction.
Our bespoke packages of care and support are based on
individual assessed needs and focus on enabling young adults to develop independence, on person-centred life skills, communication, social skills and choice.
What we offer
- 24-hour shared support, including night support
- Life and vocational skills training
- 1:1 support to complement individual assessed needs if required, wherever possible reducing support hours as skills develop and independence grows
- Comprehensive person-centred support, planning of care needs and risk assessments
- Individual healthcare planning and facilitation
- Total Communication for Service Users with Autism and or communication needs, including using the My Choice My Life visual workbook to express likes and dislikes and support with making choices
- Speech and language therapy including, where required, promoting use of pictorial and sensory aids and objects of reference to enhance communication and choice-making
- Positive behaviour management strategies, aimed at enabling people with challenging needs to substitute more appropriate behaviour
- Highly motivated, consistent, fully trained staff
- Support structured to ensure the individual has opportunities to access activities in the community e.g. football, cinema, swimming and clubs and, where possible, employment opportunities and education
- Outreach support for Service Users moving on to supported living, to provide continuity of support
- Established supported living services locally, promoting consistency and continuity in the high standard of support we provide

- Staff team with specialist training in Autism and management of challenging behaviour
- Person-centred Planning Facilitator
- Promoting self-advocacy and supporting access to advocacy services
- Promotion of employment opportunities
- Vocational opportunities and access to colleges
- Facilitation with healthcare appointments and full healthcare plan
- Initial orientation to new surrounding and establishing new routines
- Established links with local community learning disability team and healthcare professionals
- Established links with local housing providers, ready for move on
- Emotional support/socialisation
- Consistency of specific management approaches in line with risk assessment or guidelines
- Personal care
- Budget planning and finances
- Promotion of independent living skills - cleaning, laundry, menu planning, shopping, cooking
- Administration of medication
- Transport training, where applicable, to further independence
- Supporting individuals to maintain a safe environment
- Benefits support and assistance with claims from a specialist team
About the Property
Rhyme House, near the centre of Sittingbourne in Kent, provides accommodation for up to 11 young people.
The ground-floor bedroom has its own access and functions as a self-contained flat, including kitchen, living area and bathroom. It is ideal for a couple who wish to live together or a young person who has gained independent skills and is in the final stages of preparing to move to their own home.
The bedrooms upstairs have larger kitchenette and living areas and are perfect for young people nearly ready to move on to independent living.
The communal garden is well equipped and, with input from young adults, we plan to develop a sensory garden
and mini-allotment.
There are shops within walking distance and the town centre is a ten-minute walk away. Sittingbourne has good public transport links to Maidstone, Ashford, Coastal Towns and London and local amenities include a cinema, pubs, restaurants, bowling complex, colleges, swimming pool, gym, library and churches.
We also have access to a working farm for people with Learning Disabilities, in a local village, which is used successfully by Regard Service Users living elsewhere in Kent.
