Supported Living Service
The service is for adults with mental health /learning disabilities. We also work with clients experiencing early onset and vascular dementias. The service is currently a minimum support service delivering up to 4 hours of support per day tailored to the priority needs identified. There is also an on call service for out of hours. The team consists of Support Workers, Senior Support Workers and is overseen by a Service Manager. Service Users are supported to live as independently as possible using person centred planning.
A trained staff team support the service each day. Staff are trained in all areas ranging from safeguarding, health and safety, infection control to more specialist training such Autism, Deaf/blind awareness and challenging behaviour. Independence is promoted to establish a solid base for integration into the community. Service users have tailor made support plans which are used to support them with all aspects of their care and support.
