Gordon Avenue

Gordon Avenue is a supported living, younger person’s transition service, ideal for young adults leaving residential college or foster care/ family home. We specialise in supporting younger adults with learning disabilities, autism, emotional needs and or underlying mental health needs. The service can accommodate six tenants.

Our aim is to support individuals to gain the skills necessary to move on to independent living, if they so choose. We have the scope to provide outreach support once tenants have moved, thus maintaining consistency and continuity. We promote enablement and inclusion at all times giving tenants the opportunity to explore new experiences and make lifestyle changes based on their own decisions.

Staff support is flexible and structured around individual’s needs on a daily basis and the staff team are recruited specifically to meet the skills and qualities of the people they are to support. Staff will actively support and encourage individuals to attend college, seek employment both paid and voluntary and participate in numerous recreation opportunities available within local community. We have supported a number of tenants at Gordon Avenue to seek voluntary and or part time employment in specific areas of personal interest.

We have introduced an independent living skills assessment tool with tenants within Gordon Avenue which is in easy read format and outcome driven. The tools start with an person centred assessment of the individual’s skills and focuses on what skills are needed to prepare for independence. Not only does it focus on skills for independence, it also covers topics such as health, relationships, employment and socialisation. Feedback from tenants is positive and success can be measured by a notable increase in tenants self-esteem, confidence, emotional well-being and participation with daily living and social opportunities. Our aim is replicate this success and user led focus within Gordon Avenue.

Success Stories

Gordon Avenue is a semi-detached house, within walking distance of Camberley.

Located on the ground floor we have two bedrooms one with jack and jill bathroom, a kitchen/dining area with a lounge and conservatory as an extended living area.

Four further bedrooms located on the first floor with two bathrooms one with a walk in shower and one with a bath.

There is a decent sized garden at the rear of the property with BBQ area. This will become a hub of social activity during the summer months.

The home is within close proximity to shops, pharmacy, post office, pubs, church, Health Centre and parks. Public transport links to neighbouring towns and London are very good.

All tenants we will have opportunity to engage in social activities which will range from: college, employment, drama, football, bowling, cinema, swimming, gym, shopping and the numerous social and recreational opportunities available within Surrey.

The team at Gordon Avenue will provide high quality support which is tailored to reflect individual need. This is achieved through our ‘Outcome Focused Person Centred Plans’ and we closely follow the Government’s ‘Valuing People’ criteria which ensures that we respect tenant’s rights, inclusion, choice, and independence at all times.

We Offer:

24 hours a day shared support; which currently includes sleep-in support.

1:1 support for specific identified assessed needs.

We will evidence reduction in 1:1 support hours as tenants gain new skills and greater independence.

Support is structured to ensure that the individual has the opportunity to access a full range of social activities within the community.

A well motivated and enthusiastic trained staff team, trained in NVQ, Supported Living, Epilepsy, Mental Health, Challenging Behaviour, Autism and all the core mandatory training requirements. Specialist training will be provided to meet individual assessed need if required.

Support individuals in accessing appointments with all health care professionals.

We have positive working relationships with the local authority housing department, Care Managers and Health Professionals from Community Learning disability teams.

We have the scope to provide outreach support for tenants living in community as we have an established domiciliary care agency supporting children and adults which is based in Camberley

Kylie Oakden the service manager holds a Higher Diploma in Learning Disabilities, a Level 4 Registered Managers Award and Adults level 4 Health and social Care qualification. With over 20 years of working within the care sector 10 years in Australia and 10 years in the UK she has accumulated an extensive degree of experimental learning and reflective practice in the delivery of effective and efficient care for people within social and health care.

The staff team are motivated, focused and enthusiastic. They use person centred and active support to lead the way, enhancing the quality of lives for supported people. The staff team have over 10 years of experience between them and are trained in Epilepsy, Autism (Two staff members are currently under taking the Autism Champion Training), Mental Health and Challenging Behaviour, Staff have also undertaken and completed all the core mandatory training requirements

Mickaela finds a job after graduating from Foxes Academy

Mickeala from Gordon Avenue graduated from Foxes academy in July 2015. Fox’s is a residential college which also operates as a hotel. It offers training in independent life skills in residential learner houses, plus the opportunity to learn catering skills in a real operating hotel, with paying guests, where a vast selection of functions take place all year round.

Reason for nomination: Chris is manic depressive and was diagnosed bi-polar. Kylie involved him in the recruitment process of the new team (5 staff) last summer.
