The service is able to support individuals from the age of 17 as part of a transitional model of support we promote at Regard. In each case we would work closely with all parties to ensure an agreed approach is put in place to help an individual make that transition in to adulthood a positive one. Specialism: Supported living service for young people with learning disabilities and or autism in transition from residential college, foster care or the family home. Details of Vacancies: one self contained flat and two ensuite bedrooms available. Type of Service: Supported Living Age Range: 18 + years Current profile of service: The property is a large detached bungalow in Chatham within walking distance of local amenities. The service has been completely refurbished to a very high standard and has five large ensuite bedrooms plus communal areas. In addition to a large kitchen the service has a lounge and dining room as well as a secure private garden. Focus of service: Promoting independence and supporting young people to access further education, employment, social and recreational activities. Person-centred transition plans are developed for each individual to develop their budgeting skills and to become more independent in all areas of their lives. Hours provided: Staff are available 24 hours a day, including a waking night staff member. This service would suit people moving from residential care or their family home.

  • Full inclusive needs support planning, care needs and risk assessments by a well trained staff team, experienced with working with adults with Learning Disabilities and Mental Health needs.
  • Our aim is to decrease 1:1 support hours as skills are developed and assist with greater independence gained where possible
  • Support is structured to ensure that the individual has the opportunity to access a full range of activities within the community including employment opportunities and access to local colleges where possible.
  • An energetic and enthusiastic, trained staff team.
  • Supporting individuals in accessing appointments with all health care professionals.
  • Outreach support for those able to move on providing continuity of support.
